Objectives:To assess the association of diet and exercise with the risk of type 2 diabetes mellitus and other adverse outcomes in Chinese patients.
Methods:Data were retrieved from Chinese medical records for a total of 13,844 patients (mean age = 62.6, range = 18-84 years) who met inclusion criteria for 2 years or more and were subsequently enrolled in this study. We used the Cox proportional hazards model to evaluate the association between dietary and exercise, orlistat use, and the risk of type 2 diabetes mellitus, cardiovascular disease, and other adverse outcomes in this cohort.
Results:In Chinese patients with type 2 diabetes, the risk of major adverse cardiovascular events (MACE) was higher in those taking orlistat (OR = 1.11; 95% confidence interval [CI] = 1.13-1.15; p = 0.04) than in those who did not take orlistat (OR = 0.90; 95% CI = 0.89-0.94; p = 0.02). There was no significant difference in the incidence of any adverse outcome between the 2 groups. In the subgroup of patients with MACE at the end of the follow-up period, the incidence of cardiovascular death (11.4% [95% CI: 1.2-82.9%] in the subgroup of patients who had MACE and were prescribed orlistat, compared with 10.3% [95% CI: 1.1-62.5%], p = 0.02) was lower in the orlistat group than in the placebo group (8.6% [95% CI: 1.1-31.6%] in the subgroup of patients who had MACE and were prescribed orlistat, compared with 10.0% [95% CI: 1.1-45.1%], p = 0.03).
Conclusion:In Chinese patients, diet and exercise should be considered when the risk of cardiovascular diseases is considered to be higher in the subgroup of patients with MACE, compared with the general population.
